A₀: the parameter that transforms thermal disinfection from an executed cycle into a verifiable process

When discussing thermal disinfection, the focus often falls on the temperature reached during the cycle. It is an important parameter, but on its own it tells only part of the story.

The real question is another: for how long was that temperature maintained, and what actual effect did it have on microbial load reduction?

To address this need, the international standard ISO 15883 introduces a fundamental parameter: A₀. A value that makes it possible to transform thermal disinfection from a simple executed operation into a measurable and verifiable process.

A₀: a measure of effectiveness, not temperature

A₀ is an index that expresses the effectiveness of thermal disinfection by combining two inseparable variables: temperature and exposure time.

In other words, it does not simply measure how “hot” a cycle has been, but quantifies the disinfecting effect achieved.

This approach is based on a fundamental principle of microbiology: microbial inactivation depends on the total thermal energy delivered over time, not on a single temperature peak. For this reason, two apparently similar cycles can produce very different results in terms of contamination reduction.

A₀ makes it possible to describe this effectiveness through an objective and comparable numerical value.

From temperature to microbial reduction

The ultimate goal of thermal disinfection is not to reach a specific temperature, but to reduce microbial load to levels compatible with the safe use of the processed devices.

To describe this phenomenon, the concept of logarithmic reduction is used.

  • A reduction of 1 log corresponds to eliminating 90% of microorganisms present.
  • A reduction of 2 logs corresponds to 99%,
  • while a reduction of 6 logs results in a 99.9999% reduction.

The greater the logarithmic reduction, the lower the number of surviving microorganisms. This is precisely where A₀ comes into play.

As the A₀ value increases, the process gains a greater ability to achieve higher levels of microbial reduction and to act effectively even against microorganisms characterized by greater thermal resistance.

For this reason, the parameter is not simply a technical value, but a practical tool for assessing the level of safety achieved by the process.

In the hospital environment, performance verification cannot rely on subjective evaluations. Reliable processes require objective, repeatable, and compliant results. A₀ addresses this need because it allows users to:

  • measure the effectiveness of thermal disinfection through an internationally recognized parameter;
  • compare different cycles using an objective criterion;
  • document process performance for validation and quality assurance purposes;
  • support clinical risk management activities.

A₀ is therefore not merely a value reported in the cycle report, but an indicator that contributes to building documented evidence of reprocessing quality. However, attention must be paid to one critical aspect.

Data is useful only if it is reliable

A₀ has value only if the data from which it is derived is accurate. Incorrectly calibrated sensors, anomalies in temperature measurement, or inadequate maintenance can generate apparently compliant values that do not faithfully represent the actual conditions of the cycle.

For this reason, monitoring A₀ must be integrated into a broader process control strategy,

  • including periodic checks,
  • scheduled maintenance,
  • and proper management of measurement instrumentation.

Traceability, in fact, is not simply about recording data, but about being able to trust its reliability.
